We strongly believe that vaccines are a critical component to the long term health of your children. They promote health and wellness and protect our community from serious disease.

ROUTINE VACCINATIONS

Is your child coming in for a well visit soon? Here is a list of immunizations you can expect at your child’s visit:

Visit the Centers for Disease Control online to learn more about immunizations, including school and state laws, and the purpose of vaccines and how they can benefit your child.

We encourage all of our patients to follow the CDC-recommended vaccination schedule, but are available to discuss vaccine concerns with our patients.  If you decide that you do not want to vaccinate your child you must sign a vaccination refusal form to be kept on file.  If you refuse to sign the form, unfortunately we will not be able to see your child for their appointment. 

*Parents please note that if your child is under the age of 18 and they are due for vaccines, you must accompany them to their appointment to sign off on the vaccine consent form.
