Policy for Form completion: Our office encourages you to bring any form that needs to be completed at the time of your appointment. Please let the office know that a form has to be completed. There is no extra charge for that.

Form fee: Due to the fact that it takes a lot of time and resources to complete the form at a different date, we do charge a nominal fee of $5. To avoid this fee please bring the forms at the time of visit, or ask our office staff to provide you with a generic form which we keep at the office.

Requests for prescription refills or a referral can be made online by completing the form(s) below and clicking "Submit." Please allow 48 hours to process your prescription refill and up to five days to process your referral request. Thank you.
